A bit pricey, but well worth it. Can lie down in bed or on a sofa to use. The optional heat is a nice feature. Can also sit on the sofa or in a recliner chair and place this pillow where you need it (on your head, neck, shoulders, low back...). If the pressure is too strong (if you're lying prone on the bed or sofa), you can place a hand towel over the pillow for a lighter pressure.

My husband just purchased this for me. I am someone who suffers from chronic neck and shoulder pain, and I get a massage every two weeks. I was hesitant to purchase this item after reading these reviews, but one day I was hurting really bad, so he went out and bought it. I was not expecting too much based on other reviews, but I must say that I am impressed! I love it, and the best part is that it is inexpensive and it doesn't get tired! I will warn you that the shiatsu pressure can be too much for someone who is not used to massage. But the vibrating cushion massagers just don't do anything. If you don't expect the world from it, you'll be fine
I've been pleased with this massage pillow. The key is to keep your expectations realistic. Nothing will beat (or come close to) a personal deep tissue massage, but how often do you get to have that? Pros: you get some massage when and where you want for as long as you like (though you do have to restart it after 20 minutes); the heat option is nice without getting hot. Cons: a touch heavy (you always think a pillow should be light; this isn't), it's not exactly silent, and it takes some patience and/or other pillows to get it positioned comfortably.
